CSH Announces Social Innovation Fund Grantees – Integrating Housing and Health Care Demonstration

Yesterday, the Corporation for Supportive Housing announced the recipients of Social Innovation Fund grants to launch a national demonstration project on integrated models of supportive housing connected to health care. Four organizations were selected by CSH through a request for proposal process: Tenderloin Neighborhood Development Corporation of San Francisco, Economic Roundtable of Los Angeles, Connecticut AIDS Resource Coalition of Hartford, CT, and Catholic Social Services of Washtenaw County of Ann Arbor, Michigan. All of these projects will work to house and support high cost users of crisis health services through integrated housing and health care models, both improving the health status of these individuals and reducing public costs to the health care system. In total, these grantees will provide housing and health care to more than 500 individuals.
